ICF/DD-H OR ICF/DD-N PLACEMENTS
We will admit ambulatory and non-ambulatory intellectually disabled clients to our ICF/DD-H and ICF/DD-N facilities based on eligibility for services and availability of open beds.
Clients will be admitted to the facility without regard to the religion, ethnic origin, sex, degree of disability, or ability to pay. The facility shall accept and retain only those clients for whom it can provide specific programming and care.
Referral from Regional Center: The Regional Center is the placement agency for our facilities. When a potential client or his/her family is interested in admission, they need to notify the regional center and the regional center will refer the client to our facility for possible admission. The regional center that JonBec works with is Inland Regional Center. Inland Regional Center is located at: 1365 South Waterman Avenue, San Bernardino, CA 92408 and they can be reached at (909) 890-3000.
Pre-Admission Screening: JonBec will conduct a preliminary evaluation based on the placement packet of information submitted from the regional center. The preliminary information must contain background information as well as currently valid assessments of functional developmental, behavioral, social, health and nutritional status to determine if the facility can provide for the client’s needs and if the client is likely to benefit from placement in one of our facility’s.
Clients eligible for admission to the facility need to meet current admission criteria (based on Federal and State regulatory requirements) as follows:
Require intensive programming (active treatment) in one (1) of the following skill areas:
Self-Help Program: an eligible client is one who has no or minimal self-care skills and requires assistance in carrying out all activities of daily living, or has some self-care skills but requires monitoring or assistance and individualized training to maintain or improve self-care skills. Programming for clients requiring assistance in self-care skills shall include training in at least two (2) of the following:
Self-feeding
Habit training
Bowel and Bladder control
Dressing
Grooming and Hygiene
Nursing Service: an eligible client is one who requires intermittent nursing supervision by the RN consultant.
Behavior Shaping
Socialization Training
Face to face visits: Once the facility has determined that the client is eligible for admission, the facility will schedule an Admission meeting to evaluate the referral information, meet the potential client and family, and give the potential client and family a tour of the facility.
The Admission Committee comprised of the Administrator, Executive Director, QIDP, Nurse, Regional Center case manager, parents, client (wherever possible) and other members of the ID team will confer and determine whether or not the placement is appropriate.
At the time of the Admission meeting, the committee ascertains whether or not the potential client has the appropriate funding. If he/she does not have appropriate funding in place, the parents or authorized representative are requested to obtain the necessary paperwork to receive funding. If the parents/authorized representatives are not able to have funding cover the costs than the client will be considered a “private pay” client and the parent/authorized representative will be responsible for paying all expenses incurred by the client.
Admission Scheduled: If everyone is in agreement of the placement based on the Admission Meeting and visits, a date will be scheduled for the client to be admitted to the facility.
